Palliative Care

Advanced Care Planning

Advance Care Planning (ACP) is a process of often several voluntary discussions between an individual and any health care professional who knows them.

Personalised care and support planning starts with a conversation to discuss and assess your health and wellbeing in relation to your cancer.

Cancer Prehabilitation and Rehabilitation

Starting cancer treatment can be daunting, you may feel you have little control. 

There are things you can do before you start. These can include making small changes to your everyday life to help get ready for cancer treatment. This is called prehabilitation, or prehab. Recovery during and after treatment is called rehabilitation, or rehab. 

Prehab and Rehab are often supported by physiotherapists, occupational therapists, dieticians, speech and language therapists, and support workers.
